Insurance & Direct Access
Bull Dawg Athletic Training and Physical Therapy is an out-of-network provider for all health insurance except Medicare.

Out-of-network means that our trainers and physical therapists do not have a contract with your health insurance plan provider, which allows us to create a plan of care that’s flexible and specific to your individual needs to help you recover faster. Your visit with a physical therapist will be 50 minutes of uninterrupted one-on-one care.
Your reimbursement depends on your particular insurance company; most require that you meet a deductible before they will compensate you. To familiarize yourself with out-of-network benefits on your particular insurance plan, you can call the member information number on the back of your card to speak with a representative at your insurance company.
We provide you with an invoice that includes your treatment and diagnostic codes which you can submit to your insurance company for reimbursement. Payments in full are expected at the time of your visit.

We are a participating provider of Medicare, which means we will accept the Medicare-approved amount as full payment, less any co-pay or deductible that is owed by you. We also accept supplemental policies.
If you are covered by Medicare, you will need a prescription for physical therapy from your medical doctor. A referral from a different health professional, such as a dentist or chiropractor, will not be accepted by Medicare. Once you have received a prescription, it will expire for use in 30 days, so please schedule your first appointment within that 30-day time period.
Medicare requires that you have a physician order for your charges to be covered and processed. We will need a new order for physical therapy every 90 days if you are still in treatment. We will remind you when the new order is due.

In the state of Virginia, patients are able to directly access a physical therapist without a physician’s referral or prescription for physical therapy. Under “Direct Access” regulations, if you are in need of physical therapy treatment from a licensed physical therapist, you may come in for an evaluation and up to 30 days of treatment before needing a prescription from your medical provider.
Please note that some insurance carriers continue to require a referral before they will pay for physical therapy services. Contact us to inquire which insurance companies do require a prescription or whether your plan will pay for direct access.

Patients who need to cancel an appointment with Bull Dawg Athletic Training and Physical Therapy MUST DO SO 24 HOURS PRIOR TO THEIR SCHEDULED APPOINTMENT. Failure to provide a 24-hour notice of cancellation will result in a fee, which is not covered by insurance; the patient is fully responsible for the payment.
